Graph Captions / Labels need improving

Description

The graph captions need improving in the following ways

  • Traffic Rate, should label the two parts In / Out rather than ifInOctets / ifOutOctets
  • Graph titles should have the hostname and graph title in them, rather than this being text outside the graph
